1. What Is Bypass Surgery?
Coronary artery bypass grafting, also called bypass surgery or CABG, creates a new route for blood around a narrowed or blocked coronary artery.
Coronary arteries carry blood to the heart muscle. Plaque can narrow them. Some people get chest discomfort. Others have a heart attack. During CABG the surgeon uses a blood vessel from elsewhere in the body to bypass the narrowed segment. The graft may come from the chest, the arm or the leg. Why bypass may be recommended
CABG may be considered when coronary disease is better treated with surgery than with medicine alone or with a catheter. Examples include complex disease in several arteries, significant left main disease, some people with diabetes and multivessel disease, some people with reduced heart function, and anatomy that angioplasty is unlikely to treat well. Emergency surgery, extra treatment, or services outside the package can be charged differently. 3. What Is Included in a CABG Package?
There is no single definition of a bypass package. Some hospitals bundle a stay. Others itemise.

Why ICU days matter
After bypass, people usually spend time in a closely watched unit, often a cardiac ICU. How long depends on the operation and the recovery. A package may include a set number of days. Extra days are often extra. Ask the daily charge, and whether medicines and tests during those extra days are still inside the cap.
Medicines and consumables
If use goes over the allowance, the extra is billed. Ask whether routine medicines, antibiotics, blood products and extra tests are inside the estimate. The words “CABG package” do not mean every line is included.
4. What Changes the Bypass Bill?

1. How complex the coronary disease is
The number and position of narrowings, heart function, previous surgery and other heart problems change the plan.
2. Planned, urgent or emergency
A planned operation can be prepared. An emergency admission can add monitoring and a more difficult recovery. Both the risk and the bill can change.
3. How many grafts, and which vessels
The surgeon uses as many grafts as the anatomy needs. The count is not a price list and it is not a quality score.

4. On-pump or off-pump
CABG can be done with a heart-lung machine, or, in selected people, without it. The surgeon chooses from the anatomy and the condition. The two are not interchangeable for everyone. Ask which technique is proposed and whether it changes the estimate.

5. Room category
A general ward, a shared room, a single room or a suite can change the package, not only the bed charge.
6. Other illnesses
Diabetes, kidney disease, lung disease, anaemia and frailty can add tests and days.
7. Another procedure in the same admission
Bypass plus a valve, or another heart operation, is not isolated CABG. Heart valve replacement is a separate listing, at US$7,000–US$18,000, typically 8–16 nights. 8. A complication or a longer stay
Packages often assume an uncomplicated course and a defined stay. Infection, another operation or a long ICU stay can raise the bill. Ask the daily charge for extra care. The point is to understand the bill, not to predict a complication.
5. Standard, High-Risk and Repeat Bypass
Some hospitals publish different packages for a standard bypass, an emergency or high-risk case, a repeat bypass, and a combined operation. The labels mean different resources. They do not mean every “high-risk” patient will have a complication. Ask what the label means in clinical words, and which rules decide who fits the standard package.

6. Does the Number of Bypasses Set the Price?
A single, double, triple or quadruple bypass is a rough way of saying how many targets are bypassed. The plan follows the angiogram. A higher count does not by itself mean a worse outlook, and it does not prove one hospital is better.
Ask how many grafts are expected, which arteries, which graft vessels, whether the plan can change once surgery starts, whether the estimate assumes a number, and whether a change changes the package. Choose the operation the anatomy needs, not a graft count that matches a price.
7. Government Hospitals, Private Hospitals and Insurance
Government and private hospitals can differ in charges, who qualifies, and how long the wait is.
Some government hospitals provide cardiac surgery at subsidised rates or under a public scheme. What you pay depends on the hospital, eligibility and the scheme rules. Ask whether that hospital does the operation you need, whether you qualify, which package applies, whether medicines, tests and ICU are inside it, what is extra, and what the wait is. Private hospitals may offer a package or an itemised estimate. The price can move with the room, the surgeon, the complexity and the ICU. An international desk may review records and send a preliminary estimate. That estimate can change after you are examined.
Compare the surgical plan, the team's experience with this operation, the ICU, emergency support, follow-up and the written estimate. A lower price is poor value if the services you need are excluded.
Insurance coverage for CABG in India depends on the policy. Network hospital, room limit, waiting period, exclusions, co-payment and pre-authorisation all matter. 10. Before and During the Operation

Ask which tests are still required, whether the angiogram images have been seen, what to do with current medicines, when to stop eating if you are told to, what to bring, the expected stay, who explains consent, and the main risks in your case. Do not stop aspirin, an anticoagulant, a diabetes medicine or any other prescribed drug unless the treating team says so.

The plan may include monitoring, pain relief, wound care, gradual walking, breathing exercises, a medicine review, and discharge planning. A longer stay can add room charges, medicines, tests, nursing or ICU time. Ask how extra days are billed before the operation.

Items that are often outside a defined package include ICU beyond the included days, extra ward days, medicines or tests above the cap, blood products, extra specialist visits, organ support if it is needed, treatment of infection, a further procedure, rehabilitation, and the medicines you take home. Not every patient needs these. Ask which are excluded, and for a rate where the hospital can give one.
12. How to Compare Two Estimates
Use the same questions for both hospitals. Confirm the exact operation, whether surgeon and anaesthesia are included, how many ICU days, the room and how many ward days, test and medicine caps, whether blood is included, the charge for an extra day, how a complication is billed, which follow-up is included, and the written total. Two estimates that both say “CABG” can still differ. Also compare whether the team and the hospital can manage this anatomy and a complication. 13. Bypass and Angioplasty Are Not the Same Bill
Angioplasty is usually less invasive and the first recovery is often shorter. CABG is a major operation with a longer recovery. For some people with complex multivessel disease, bypass can offer longer-term benefits, including fewer repeat procedures, than a stent strategy. For others, angioplasty is the appropriate treatment. A lower first price is not automatically a lower cost over time. Repeat procedures, medicines and recovery all count.

Ask who will operate, what their training and experience with CABG are, whether the hospital does this operation regularly, whether a cardiac ICU is available, what happens if there is a complication, which other specialists are on site, what rehabilitation and follow-up exist, whether you will get a written estimate with exclusions, who explains the medicines, and how records will reach a doctor outside India.
