Delhi NCR holds the densest concentration of tertiary hospitals in northern India. Three things follow from that for anyone travelling here for urology, plus one thing worth planning around.
1. Subspecialists Are Common Rather Than Rare
If you need a surgeon who does only reconstruction, or only stone work, or only paediatric urology, you can usually find one here without compromising. In smaller cities you often take the best available generalist instead. That is not always the wrong choice, but it should be a choice rather than the only option on the table.
2. The Easiest Arrival Point in India
Indira Gandhi International handles direct flights from most of West and East Africa, the Gulf, Central Asia and Europe. For many patients that means one flight rather than two, with no overnight in a third country. If you are travelling while unwell, or with a child, that difference is worth more than it looks.
3. Cancer Cases Go to a Board
Where the diagnosis is cancer of the prostate, bladder or kidney, the plan should come out of a multidisciplinary meeting rather than from one surgeon. The urologist should be sitting down with medical and radiation oncologists, radiology and pathology.
Ask whether your case goes to that meeting, and ask for the conclusion in writing. The large NCR centres run them as routine, so the question should be easy to answer.
4. Timing and Air Quality
Air quality in Delhi NCR is poor from roughly late October to January. For most urological surgery this changes nothing, but it is worth raising with your surgeon if either of these applies:

You face a long operation under general anaesthesia
May and June are very hot, which matters more for recovery outside hospital than for the surgery itself. February to April and September to October are the most comfortable months to be here. None of this justifies delaying urgent treatment.
Language
Consultations run in English at the major hospitals, and Hindi is the local language rather than one you will need. Interpreters for Arabic, French, Russian and Kiswahili are available at most of the hospitals in this directory, booked before you arrive rather than found on the day.
Cost
Cost is usually lower than in Europe, North America or the Gulf, though how much lower depends on the procedure, the technique and how long you stay.

About Urology Care in Delhi NCR

1. Delhi NCR Is Not One City
It covers Delhi itself plus four surrounding cities, and the hospitals are spread across all of them:

A hospital described as being in Delhi may sit forty kilometres from the one you were comparing it against, in traffic that turns that into two hours at the wrong time of day. Before you book anywhere to stay, find out which part of the region your hospital is in and book near that.
2. Stone Disease
This is the largest share of urology work here, as it is across northern India. Most of it is treated endoscopically, and the choice between shock waves, a telescope passed up the urinary tract, and a small tract made directly into the kidney depends on the size and hardness of your stone and the shape of your kidney.
Stones sitting in the bladder rather than the kidney are a different problem with a different fix, usually bladder stone removal through the urethra. In men they are often a sign that something downstream is obstructing the flow. Treating the stone without treating the cause means seeing it again.
3. The Prostate
An enlarged prostate is not cancer, though the symptoms overlap enough that assessment starts with examination and tests. When medication is no longer enough, surgery is usually resection through the urethra, the operation called TURP, or one of several laser alternatives.
Where cancer is the question rather than obstruction, the answer comes from a prostate biopsy and not from a blood test alone. A raised PSA has several explanations, most of them not cancer, which is why a single number should never be the whole basis for a decision.
4. Reconstructive Urology
This is one of the areas where the NCR's depth of subspecialisation shows. Narrowing of the urethra after injury, infection or previous instrumentation is treated by urethral surgery, and the results depend heavily on how much of this particular work the surgeon does.
This is not a procedure to have from someone who performs it occasionally. Ask directly how many they do in a year.
5. Male Fertility and Sexual Health
The specialty also covers male fertility, erectile dysfunction and Peyronie's disease treatment. These are ordinary medical problems with established treatments and nothing to be awkward about raising.
Go to someone who does this work regularly. This corner of the field attracts a good deal of unregulated advertising.
6. Paediatric Urology
This is a separate practice needing separate training. Conditions like an undescended testicle need a surgeon who operates on children routinely, and undescended testicle surgery has a timing element that matters, so it is worth asking about early rather than waiting.
An adult urologist, however senior, is not the right person for a child. If you are looking for a paediatric urologist specifically, say so at the outset so you are not sent to a general adult clinic.
