- Over time, lipid-laden plaques trigger inflammatory cascades, fibrous cap formation, and varying degrees of luminal narrowing — quantified angiographically as percent diameter stenosis.
- Hemodynamically significant stenoses (typically ≥ 70% in non-left main vessels, or ≥ 50% in the left main coronary artery) reduce coronary flow reserve, precipitating myocardial ischemia under stress or, in the setting of acute plaque rupture, causing unstable angina or acute myocardial infarction (AMI).
