What are the symptoms and health risks of Uterine Prolapse?
Common symptoms include a sensation of pressure, heaviness, or 'something falling out' in the pelvis, lower back discomfort, difficulty with urination or bowel movements, and, in more advanced cases, visible or palpable tissue at or outside the vaginal opening. Some women also report pain or discomfort during sexual intercourse, or notice that symptoms worsen with prolonged standing or physical activity and ease when lying down. It is important to note that the presence or severity of symptoms does not on its own establish a clinical diagnosis — a formal examination by a qualified clinician is required.
How is Uterine Prolapse diagnosed?
Diagnosis is typically made through a pelvic examination, during which the clinician assesses the degree of descent using a standardised grading system; the most widely used is the Pelvic Organ Prolapse Quantification (POP-Q) system, which classifies prolapse from Stage I (mild descent, remaining above the vaginal opening) to Stage IV (complete prolapse outside the body). In addition to examining the uterus, the clinician will assess for concurrent prolapse of the bladder (cystocele) or rectum (rectocele), as these frequently co-occur. Mild to moderate prolapse is frequently managed without surgery through pelvic floor rehabilitation, lifestyle modification, and the use of a vaginal pessary to provide mechanical support. When prolapse is more advanced, symptoms are significantly affecting quality of life, or conservative measures have not provided sufficient relief, surgical options — which may include uterine-preserving repair procedures or hysterectomy combined with pelvic floor reconstruction — may be considered.
Can Uterine Prolapse be treated without surgery?
Non-surgical management is a well-established and effective pathway for many women, particularly those with Stage I–II prolapse or those who are not surgical candidates. Supervised pelvic floor muscle training (Kegel exercises), management of contributing factors such as chronic constipation or excess weight, and fitting of a vaginal pessary (a removable device that mechanically supports the uterus) are the principal non-surgical options. Topical oestrogen therapy may also be recommended for postmenopausal women to help maintain vaginal tissue health, though its use is guided by individual medical assessment.
