What is a nipple-sparing mastectomy?

A nipple-sparing mastectomy removes the breast glandular tissue while preserving the nipple-areola complex — the nipple and the darker skin around it — when the cancer, the anatomy and the treatment plan make that approach appropriate. The skin envelope is also kept. The intent is still oncological: to clear disease in the breast. Keeping the nipple is not a cosmetic upgrade you can buy independently of those facts.

In selected patients the nipple can be spared without leaving tumour behind, which also helps later reconstruction. Sensation is often reduced or lost. Colour change and, uncommonly, partial or complete nipple loss from poor blood supply are recognised risks and belong in the consent.

Suitability depends on tumour location (especially distance from the nipple), the absence of clinical nipple involvement, breast size and droop, prior scars or radiation, and the wider oncology plan. Inflammatory breast cancer, Paget's disease of the nipple, bloody nipple discharge with proven malignancy, and some central tumours are typical reasons a surgeon will not offer this operation. Reconstruction is frequently discussed in the same week — an implant, expander or flap — but it is a second operation even when it shares an anaesthetic. Lymph-node surgery is usually part of the same sitting: sentinel node biopsy when nodes look clear, or axillary dissection when nodal disease is already known.
