What is robotic knee replacement?

In a robotic (or robotic-assisted) knee replacement, the surgeon still performs the operation — the robot doesn't operate on its own.

Beforehand, a scan-based 3D model of your knee is built. During surgery, a robotic arm helps the surgeon make the bone cuts and position the implant to that plan, within very fine margins.

The whole point is precision: aligning and balancing the implant as closely as possible to the plan.

How robotic surgery differs from conventional

Conventional knee replacement uses manual jigs and cutting guides, and relies on the surgeon's eye and experience — which, in skilled hands, is already very good.

Robotic surgery adds real-time data and a physical guide that holds the cuts to the plan, which is why it produces fewer alignment "outliers".

The trade-offs: robotic cases tend to take a little longer, use extra pin sites, and cost more.

What does the evidence actually show?

This is where honest information matters, because a lot of marketing oversells robotics.

Large reviews of randomised trials agree on one point: robotic surgery is more accurate at implant alignment and produces fewer alignment outliers than conventional surgery.

What they don't show, so far, is a consistent improvement in what patients care about most — pain, function, satisfaction and how long the implant lasts. Over the medium term those tend to be similar to conventional surgery.

Some studies report slightly less early pain or a shorter early stay; others find no difference. One large 2026 study even found a higher rate of early major complications with robotic surgery, linked to the extra pins and longer operating time.

The honest summary: robotic surgery reliably improves precision, but its long-term superiority is still unproven. It's a tool used well by the right surgeon — not a guarantee of a better knee.

Is robotic better than conventional?

For most straightforward knees, a skilled surgeon achieves excellent results either way, and conventional surgery has decades of proven track record.

Robotic assistance may matter more in specific situations — significant deformity, or anatomically difficult knees — where getting alignment exactly right is harder by hand.
