Shoulder arthroplasty carries a well-characterised risk profile that patients must understand prior to giving informed consent. The overall complication rate at high-volume centres is low (<5–8%) but includes the following specific concerns: GLENOID COMPONENT LOOSENING: The most common long-term failure mode in anatomic TSA, occurring in 5–10% of cases at 10 years. Risk is higher with posterior glenoid erosion (Walch B2/B3 morphology), soft cement technique, and glenoid malpositioning (retroversion >10°). PSI and navigation reduce but do not eliminate this risk. SCAPULAR NOTCHING: Specific to rTSA; inferior scapular neck erosion caused by the humeral cup impinging on the scapular pillar during adduction. Reported in 44–96% of classic Grammont medialized designs on long-term radiographic follow-up. Lateralised glenosphere designs reduce but have not eliminated notching. Clinical significance of Grade I–II notching remains debated, but Grade III–IV notching correlates with glenosphere loosening. PERIPROSTHETIC JOINT INFECTION (PJI): Incidence 0.5–2.0% for primary shoulder arthroplasty. Causative organisms most commonly include Propionibacterium acnes (Cutibacterium acnes) — a slow-growing anaerobe colonising pilosebaceous units of the shoulder; can present with indolent pain years post-operatively. Diagnosis requires synovial aspirate analysis, extended culture (14 days minimum on anaerobic media), serum CRP, and ESR. Management: débridement and implant retention (DAIR) for early acute infection; two-stage revision exchange arthroplasty for late chronic infection. AXILLARY NERVE INJURY: Reported in 1–4% of primary cases; most commonly a neurapraxia resolving within 3–6 months. Vigilant retractor placement during the deltopectoral approach is protective. Persistent axillary nerve palsy with complete deltoid denervation is a contraindication for rTSA and a devastating functional loss. INSTABILITY / DISLOCATION: More common after rTSA (1–4%); risk factors include inadequate soft tissue tensioning, component malpositioning, and aggressive early physiotherapy. Managed by closed reduction if acute; revision surgery for recurrent instability. PERIPROSTHETIC FRACTURE: Intraoperative calcar or greater tuberosity fracture during stem insertion (0.5–3%); post-operative periprosthetic fracture with fall (rare).
