Who is a Candidate?

Treatment Options & Approaches

Routes of leakage include: paravertebral soft tissue (usually inconsequential), epidural space (risk of spinal cord or nerve root compression — most serious), intervertebral disc space (associated with accelerated adjacent disc degeneration), and basivertebral or segmental veins (risk of pulmonary cement embolism). ADJACENT-LEVEL VERTEBRAL FRACTURE (second most clinically important risk): New VCFs at the level immediately above or below the treated vertebra occur in 12–52% of patients within 12 months of vertebroplasty, compared to 12–22% in conservatively managed VCF patients. The cement-augmented vertebra creates a stiffer biomechanical unit, increasing stress concentration at adjacent endplates. This risk is substantially mitigated by concurrent initiation of anti-osteoporosis pharmacotherapy (bisphosphonates, denosumab, teriparatide, or romosozumab) and patient education on fall prevention. PULMONARY CEMENT EMBOLISM: Clinically significant cement pulmonary embolism (confirmed on CT pulmonary angiography) occurs in approximately 0.1–0.6% of vertebroplasty procedures.
