Treatment Options & Approaches
- SIMULTANEOUS PANCREAS-KIDNEY (SPK) TRANSPLANT — STANDARD APPROACH: The SPK procedure is performed under general anesthesia via a midline or bilateral lower-quadrant laparotomy.
- The pancreatic allograft (procured with the duodenal segment intact) is implanted in the right iliac fossa with arterial reconstruction using a donor Y-graft (iliac artery bifurcation) anastomosed end-to-side to the recipient right common or external iliac artery.
- Venous drainage is achieved via end-to-side anastomosis to the right common iliac vein (systemic drainage) or, at select centers using the portal-enteric drainage technique, to the superior mesenteric vein for more physiological insulin delivery directly into the portal circulation.
