- Intestinal failure is defined as the reduction of functional gut mass below the minimum necessary for adequate digestion and absorption of macronutrients, micronutrients, water, and electrolytes to sustain life without intravenous supplementation.
- The commonest causes requiring transplant evaluation include short bowel syndrome secondary to massive resection (Crohn's disease, mesenteric vascular catastrophe, radiation enteritis, volvulus, or necrotising enterocolitis in neonates), chronic intestinal pseudo-obstruction, and extensive mucosal disorders such as microvillus inclusion disease or tufting enteropathy.
- Patients with fewer than 100 cm of residual small bowel in adults — or less than 20–25 cm without an intact colon — almost universally require long-term TPN, which carries a compounding risk of TPN-associated liver disease (TPNALD), central venous catheter-related bloodstream infections (CRBSIs), venous thrombosis, and metabolic bone disease.
