- The condition exists on a spectrum from mild, self-limiting disease to severe acute-on-chronic liver failure (ACLF), with the latter carrying a 28-day mortality of up to 30–50% without targeted therapy.
- Pathophysiologically, ethanol and its toxic metabolite acetaldehyde drive oxidative stress, mitochondrial dysfunction, gut dysbiosis with increased intestinal permeability, and a systemic pro-inflammatory cytokine cascade — most notably TNF-α and IL-8 — that culminates in massive hepatic necroinflammation and progressive fibrosis.
