- Cervical incompetence (also termed cervical insufficiency) is a structural or functional failure of the uterine cervix to remain closed and maintain adequate length during the second trimester of pregnancy, resulting in painless cervical dilation, membrane prolapse, and pregnancy loss typically between 14 and 28 weeks of gestation.
- The pathophysiology involves deficient collagen cross-linking and smooth-muscle architecture within the cervical stroma, which may be congenital (as in Müllerian anomalies or connective-tissue disorders such as Ehlers-Danlos syndrome), or acquired following large-loop excision of the transformation zone (LLETZ/LEEP), cone biopsy, cervical lacerations from prior deliveries, or uterine curettage.
- Sonographic surveillance demonstrating a short cervix (transvaginal ultrasound cervical length < 25 mm before 24 weeks) or a dilated internal os with funneling is the cornerstone of modern diagnosis, replacing the older historical and clinical diagnostic criteria.
