These symptoms overlap with many benign conditions, and their presence alone is not sufficient to establish a diagnosis; formal clinical and pathological evaluation is always required.
How is Soft Tissue Sarcoma diagnosed?
Diagnosis typically begins with a clinical examination and cross-sectional imaging — MRI is preferred for extremity and trunk tumours, while CT is often used for retroperitoneal or visceral lesions — to characterise the tumour's size, location, and relationship to adjacent structures. A tissue biopsy, most often performed as a core-needle or incisional biopsy by a specialist experienced in sarcoma, is essential to confirm malignancy and determine the histological subtype and grade (commonly reported as low, intermediate, or high grade), which strongly influences prognosis and treatment planning. Staging investigations, including CT of the chest to assess for pulmonary metastases, are standard once a sarcoma diagnosis is confirmed.
How is Soft Tissue Sarcoma treated?
Treatment is highly individualised and depends on the tumour's subtype, grade, size, anatomical location, and the patient's overall health. For localised disease, wide surgical resection — removing the tumour with a margin of healthy surrounding tissue — is the primary treatment and is often combined with radiation therapy to reduce the risk of local recurrence. In cases of high-grade, large, or borderline-resectable tumours, systemic chemotherapy may be used before surgery (neoadjuvant) to shrink the tumour or after surgery (adjuvant) to address microscopic residual disease, though the benefit varies considerably by histological subtype and is determined on a case-by-case basis by the treating team.
Can Soft Tissue Sarcoma be treated without surgery?
Unlike some cancers where surveillance or medical management alone may be appropriate for indolent disease, the vast majority of soft tissue sarcomas require procedural treatment — surgery is the cornerstone of curative intent for localised tumours, and there is limited evidence supporting a watch-and-wait approach for confirmed malignant lesions. That said, radiation therapy and chemotherapy may serve as primary treatments in specific situations where surgery is not feasible, and some low-grade subtypes in elderly or medically frail patients may be managed with close monitoring when the risks of intervention outweigh the likely benefit; this is a decision made individually by the multidisciplinary team.
