A team to help with planning, medical translation and follow-up.
What is Total gastrectomy?
Total gastrectomy is the complete surgical removal of the stomach, most commonly performed to treat gastric (stomach) cancer, followed by reconstruction that connects the esophagus directly to the small intestine. It is recommended when the tumor is large, located in the upper/middle stomach, or diffusely spread, and is often combined with lymph node dissection and, in select cases, chemotherapy. The goal is complete cancer clearance while restoring a functional digestive pathway.
Who may be considered?
Total gastrectomy may be considered after a qualified specialist reviews your reports and medical history. Suitability varies from person to person and must be assessed by your treating doctor.
How does it work?
Performed through a single abdominal incision, this traditional approach is preferred for locally advanced tumors, large masses, or cases requiring extensive lymph node clearance.
Open Total Gastrectomy
Standard ApproachPerformed through a single abdominal incision, this traditional approach is preferred for locally advanced tumors, large masses, or cases requiring extensive lymph node clearance.
Comprehensive Investigations
Upper GI endoscopy with biopsy, CT abdomen/chest with contrast, PET-CT (if indicated), CBC, liver and kidney function tests, tumor markers (CEA, CA 19-9), ECG, pulmonary function assessment

Recovery and follow-up
Patients typically resume a liquid-to-soft diet gradually over several weeks, with regular follow-up including nutritional counseling and vitamin B12/iron supplementation, since the stomach's absorptive functions are lost. Full recovery and dietary adaptation may take 2-3 months, with long-term oncology follow-up for cancer surveillance.
Risks
Potential risks include anastomotic leakage, infection, bleeding, nutritional deficiencies (vitamin B12, iron, calcium), dumping syndrome, and, in rare cases, complications related to anesthesia or delayed wound healing. Long-term dietary adjustment is required due to complete stomach removal.
Alternatives
Depending on tumor stage and location, alternatives may include subtotal (partial) gastrectomy, endoscopic mucosal resection for very early-stage cancers, or neoadjuvant chemotherapy followed by surgery; a multidisciplinary tumor board evaluation helps determine the most appropriate option.
