What is Total Body Irradiation (TBI)?
Total Body Irradiation (TBI) is a specialized radiation therapy technique that delivers a precisely calculated dose of radiation to the entire body, most commonly used as part of the conditioning regimen before a bone marrow or stem cell transplant in patients with leukemia, lymphoma, or other blood disorders. It works by destroying residual cancer cells and suppressing the immune system so that donor stem cells can engraft successfully. TBI is typically planned and delivered by a specialized radiation oncology team in close coordination with the transplant hematology team.

Recovery and follow-up

Risks
TBI can cause short-term side effects such as nausea, fatigue, mucositis, and temporary organ toxicity (especially to lungs and kidneys), along with a small long-term risk of secondary cancers, infertility, and cataracts.
Alternatives
Depending on the disease and patient fitness, chemotherapy-only conditioning regimens (without radiation) may be considered as an alternative to TBI before stem cell transplantation, though TBI is often preferred for certain leukemias due to its ability to reach sanctuary sites inaccessible to chemotherapy.
