What is Vitiligo Surgery?
Vitiligo surgery refers to a group of skin grafting techniques used to restore pigment in stable, extensive, or treatment-resistant vitiligo patches that have not responded to medical therapy or light treatment. These procedures transplant a patient's own healthy melanocytes (pigment-producing cells) from unaffected skin to depigmented areas to stimulate natural repigmentation. It is generally recommended only for stable vitiligo (no new patches or spreading for at least 6-12 months).
Who may be considered?
Vitiligo Surgery may be considered after a qualified specialist reviews your reports and medical history. Suitability varies from person to person and must be assessed by your treating doctor.
How does it work?
A thin layer of donor skin is processed into a cell suspension containing melanocytes and keratinocytes, which is then applied to the dermabraded recipient site; it can cover larger areas with a smaller donor site.
Melanocyte-Keratinocyte Transplantation Procedure (MKTP)
Most PopularA thin layer of donor skin is processed into a cell suspension containing melanocytes and keratinocytes, which is then applied to the dermabraded recipient site; it can cover larger areas with a smaller donor site.
Comprehensive Investigations
CBC, blood sugar, coagulation profile, Wood's lamp examination, skin biopsy (to confirm stability), ECG (for adults), HIV/Hepatitis screening

Recovery and follow-up
Initial healing takes 1-2 weeks with protective dressings, while visible repigmentation typically begins after 3-6 weeks and continues gradually over 3-6 months, often supported by follow-up phototherapy. Sun protection and avoidance of trauma to treated areas are essential during recovery.
Risks
Potential risks include partial or no repigmentation, color mismatch, infection, scarring at donor or recipient sites, and the Koebner phenomenon (new vitiligo triggered by skin trauma). Success rates vary depending on vitiligo stability, site treated, and technique used.
Alternatives
Non-surgical alternatives include topical corticosteroids or calcineurin inhibitors, narrowband UVB phototherapy, excimer laser therapy, and camouflage cosmetics, which are generally tried before considering surgical options for stable, resistant patches.
