What is Esophageal Variceal Banding?
Esophageal Variceal Banding, also called Endoscopic Variceal Ligation (EVL), is a minimally invasive endoscopic procedure used to treat or prevent bleeding from enlarged veins (varices) in the esophagus caused by liver cirrhosis and portal hypertension. During the procedure, small elastic bands are placed around the varices to cut off their blood supply, causing them to shrink and scar over. It is commonly performed both as an emergency treatment for active bleeding and as a planned procedure to prevent future bleeding episodes.
Who may be considered?
Esophageal Variceal Banding may be considered after a qualified specialist reviews your reports and medical history. Suitability varies from person to person and must be assessed by your treating doctor.
How does it work?
A planned outpatient or short-stay endoscopic procedure performed in stable cirrhosis patients with high-risk varices to prevent a first bleed or rebleeding, usually done in a series of sessions spaced a few weeks apart.
Elective / Prophylactic Endoscopic Variceal Ligation

Most patients can resume light activity within a few days, though a soft diet and avoidance of strenuous activity is advised for about a week; follow-up endoscopy is typically scheduled every 2-4 weeks until varices are eradicated, followed by periodic surveillance.
Risks
Potential risks include post-banding esophageal ulceration, transient chest discomfort, rebleeding, esophageal stricture, and rarely perforation or infection; patients with severe liver dysfunction also carry general anesthesia-related risks.
Alternatives
Alternative or adjunct treatments include pharmacological therapy with non-selective beta-blockers, sclerotherapy, balloon tamponade for acute bleeding, and Transjugular Intrahepatic Portosystemic Shunt (TIPS) for refractory or recurrent variceal bleeding.
