For some benign biliary strictures, particularly those caused by inflammation or early post-procedural changes, a period of careful monitoring combined with management of the underlying condition (for example, immunosuppressive therapy in primary sclerosing cholangitis, or treatment of a bile duct infection with antibiotics) may be appropriate before or instead of an interventional procedure. However, biliary strictures that cause meaningful obstruction generally do require an interventional or surgical approach to relieve the blockage and prevent ongoing liver injury — there is limited evidence that medical management alone can resolve an established mechanical stricture, and a gastroenterologist or hepatobiliary surgeon should advise on whether watchful waiting is appropriate in any individual case.

Endoscopic Retrograde Cholangiopancreatography (ERCP) may be considered only when it is appropriate for the confirmed diagnosis and the expected benefit outweighs the individual risks. The decision may depend on symptom severity, investigation results, previous treatment, overall health and the patient's goals.
