How is Endocarditis diagnosed?
Diagnosis is typically guided by the Duke Criteria, a validated clinical framework that combines blood culture findings, echocardiographic evidence of valve or endocardial involvement, and additional clinical and laboratory markers to classify cases as definite, possible, or rejected. Transthoracic echocardiography (TTE) is usually the first imaging step, but transoesophageal echocardiography (TOE/TEE) is often needed for clearer visualisation of valve structures, abscesses, or prosthetic valve involvement. Additional investigations — including serial blood cultures, inflammatory markers (CRP, ESR, procalcitonin), full blood count, and, where available, advanced imaging such as cardiac CT or PET-CT — are selected based on individual clinical presentation and the treating team's judgment.
How is Endocarditis treated?
Treatment approach depends on the causative organism, the severity of valve damage, whether complications such as heart failure, abscess, or embolic events have occurred, and the patient's overall clinical condition. Prolonged intravenous antibiotic therapy — typically four to six weeks — is the cornerstone of management for most patients and may be sufficient when valve damage is limited and there are no major complications. When structural damage is significant, when infection cannot be controlled medically, or when there is a high ongoing embolic risk, heart valve repair or replacement surgery becomes necessary, and the timing of any intervention is a decision made by a specialist team after individual assessment.
Can Endocarditis be treated without surgery?
Many patients with endocarditis are managed successfully with intravenous antibiotic therapy alone, guided by blood culture sensitivities and delivered under close inpatient or, in carefully selected stable cases, outpatient parenteral antibiotic therapy (OPAT) programmes.
